Eligibility Guidelines
Every person’s care needs are different. Some patients need medical support after an illness, injury, or hospital stay. Others need comfort-focused care during a serious illness. Some families simply need extra help at home with daily tasks, companionship, or personal care. At Compassionate Care, our team helps patients, families, and providers understand which service may be the right fit and what eligibility requirements may apply.
Home Health Eligibility
Home Health is medical care provided in the home for patients who meet certain clinical and insurance requirements.
For Medicare-covered Home Health services, a patient generally must:
- Be considered homebound
- Be under the care of a physician or allowed practitioner
- Have a plan of care established and reviewed by a physician or allowed practitioner
- Need intermittent skilled nursing care, physical therapy, or speech-language pathology
- Or have a continuing need for occupational therapy
Home Health may be appropriate after a hospitalization, illness, surgery, injury, or change in condition when skilled care is needed at home.
Our team can help review the situation and coordinate with the patient’s provider to determine whether Home Health may be appropriate.
Hospice Eligibility
Hospice care may be appropriate when a patient has a serious, life-limiting illness and their physician believes they may have a prognosis of six months or less if the illness follows its normal course.
Hospice focuses on comfort, dignity, symptom management, emotional support, and quality of life.
A patient may be eligible for Hospice when:
- They have a terminal or life-limiting diagnosis
- Curative treatment is no longer the primary goal
- Their care goals have shifted toward comfort and quality of life
- They are experiencing decline related to a serious illness
- A physician determines they may have a prognosis of six months or less
Examples of hospice-appropriate diagnoses may include advanced cancer, end-stage heart disease, advanced COPD, dementia, stroke-related decline, kidney disease, liver disease, or other serious illnesses.
In-Home Care Eligibility
In-Home Care is available for individuals who could benefit from additional non-medical support at home. This may include help with personal care, companionship, meal preparation, light housekeeping, medication reminders, transportation support, or assistance with daily routines.
In-Home Care may be a good fit for someone who:
- Needs extra help with daily activities
- Wants to remain safely and comfortably at home
- Could benefit from companionship or supervision
- Has a family caregiver who needs support or respite
- Is recovering, aging, or living with changing care needs
Because In-Home Care is non-medical, eligibility is generally based on the person’s need for support rather than a medical diagnosis.
