Job Description:

  • Provides care/services to patients including the administration of treatments and medications according to the patient’s plan of care.
  • Prepares clinical and progress notes.
  • Maintains accurate and timely documentation of care/services provided.
  • Reports any changes in the patient’s condition or plan of care to the Registered Nurse.
  • Participates in the nursing process by evaluating and making recommendations for revisions to the plan of care, as appropriate.
  • Only performs treatments or special procedures as authorized by the Home Health Agency and deemed competent to perform by the Clinical Manager.
  • Prepares equipment and materials for treatments observing aseptic technique, as required.
  • Assists the patient in learning appropriate self-care techniques.
  • Demonstrates competency in providing patient care/services and provides care/services in accordance with professional standards and the state’s nurse practice act.
  • Provides appropriate patient education and teaching materials.
  • Reviews the patient’s medications; updates the patient’s medication profile when there are new, changed, or discontinued medications; and notifies the Registered Nurse of medication changes.
  • Communicates with other team members, as appropriate, regarding the patient’s status.
  • Attends and participates in staff meetings, educational in-services, and committee meetings,
    as appropriate.
  • Participates in the on-call rotation as requested.
  • Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to the Clinical Manager.
  • Documents the patient’s progress and effectiveness in meeting goals and outcomes.
  • Participates in the patient’s discharge planning.
  • Participates in the agency’s Quality Assessment and Performance Improvement (QAPI) program, as appropriate.
  • Adheres to the agency’’s policies and procedures.
  • Maintains confidentiality of all aspects of job performance.
  • Practices proper infection control and utilizes PPE as indicated.
  • Complies with all applicable federal, state, and local laws and regulations in performance of job functions.
  • Performs other related functions as directed.

Requirements:

  • A graduate of an approved school of nursing who is licensed as an LPN/LVN by the state in which practicing.
  • At least one year of recent nursing experience preferred.

Clinical Manager

Requirements:

  • Provides skilled occupational therapy services and assessment in accordance with the physician’s plan of care.
  • Assesses patient’s functional status, establishes goals, and develops a plan of care.
  • Provides direct occupational therapy treatment to patients to improve or restore physical function, activities of daily living (ADLs), and instrumental activities of daily living (IADLs).
  • Instructs patients, families, and caregivers in the use of adaptive equipment and compensatory techniques to maximize independence.
  • Therapeutic activities to restore physical function include exercises for neuromuscular re-education and cognitive re-training.
  • Therapeutic tasks and activities to restore sensory-integrative function.
  • Treatments address upper-extremity function: functionality, mobility, range of motion and strength; safety needs; and energy conservation.
  • Compensatory techniques to improve level of independence in ADLs.
  • Designing, fabricating, and fitting orthotic and/or self-help devices to compensate for loss of function.
  • Evaluates the home environment for safety concerns and recommends home equipment.
  • Assigns and supervises COTAs as required by state regulations, if any, and accreditation standards.
  • Assigns and supervises Home Health Aides as required by Medicare/Medicaid and state regulations, if any.
  • Participates in patient care conferences as needed or required by person.
  • Demonstrates competency in providing care/services and provides care/services in accordance with the plan of care.
  • Completes appropriate competencies for Occupational Therapists and COTAs as directed by the Clinical Manager.
  • Maintains accurate and timely clinical records.
  • Communicates with the physician and other team members regarding the patient’s plan of care and progress.

    Medical Social Worker/Social Work Associate

    Requirements:

    1. Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required.
    2. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
    3. Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
    4. Maintains clinical records on all patients referred to social work prepares clinical and progress notes.
    5. Educates patients and families on, and assists in, preparation of advanced directives.
    6. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
    7. Provides information to patients and families/caregivers and community agencies.
    8. Serves as liaison between patients and families/caregivers and community agencies.
    9. Maintains collaborative relationships with organization personnel to support patient care.
    10. Maintains and develops contracts with public and private agencies as resources for patient and personnel.
    11. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
    12. Assists physician and other team members in understanding significant social and emotion- al factors related to health problems and death/dying issues.
    13. Actively participates in quality assessment performance improvement teams and activities.
    14. Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
    15. Other duties as delegated by the Clinical Supervisor/Nursing Supervisor.

    5. Provides information, referral services, and application assistance for community organizations and resources (Medi-Cal, IHSS etc) for patients and families/caregivers. Provides information and links patients or families/caregivers with community agencies
    6. Serves as liaison between patients or families/caregivers and community agencies.
    7. Maintains collaborative relationships with organization personnel to support patient care.
    8. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
    9. Participates in the development of the total plan of care and case conferences as required.
    10. Assists physicians and other team members in understanding significant social and emotional factors related to health problems.
    11. Participates in discharge planning.
    12. Other duties as delegated by the Clinical Director/Supervisor.
