If you’re unsure which service is the right fit, send us the information you have and our team will help guide the next step. To help us process the referral, please include patient demographics, insurance information, current medication list, and History & Physical visit notes from the past 90 days.
Home Health

We accept referrals across hospice, home health, and in-home care. If the right service line is unclear, please send us the information you have and we will help determine the next step.
Choose Home Health when…
- Patient is homebound with a skilled need
- Recent hospital discharge or procedure recovery
- Chronic condition management such as CHF, COPD, diabetes, or wounds
- Goals are restorative, including regaining strength, function, or returning to baseline
Choose Hospice when…
- A serious illness may have a life expectancy of 6 months or less if the disease runs its expected course
- The patient is experiencing ongoing decline, such as weight loss, eating less, sleeping more, falls, repeat ER visits, or wounds/infections that are not improving
- Goals are shifting from curative treatment to comfort, safety, and quality of life at home
- Patient or family needs more support with symptoms, medications, equipment, emotional care, or 24/7 nurse access
Consider both together when…
- Uncertain prognosis, start with Home Health, transition if appropriate
- Family needs time to process goals of care
- Palliative needs alongside active treatment
